This form is to be completed by the parent or guardian enrolling a child in the Greenland Recreational After School Program (GRASP).
Completion of the required forms does not guarantee registration / acceptance of your child into GRASP. A GRASP staff member will contact you to continue the registration process once your submissions are received.
In addition to this enrollment form, you will also need to fill out the additional documents linked below:
- The GRASP Wait List Application Form
- The GRASP Enrollment Form (this form)
- The GRASP Parent Manual
Important: This Enrollment Form is the second of three required documents.
You must have submitted the Waitlist form first and received communications from GRASP stating that you can proceed with enrollment before filling out this form.
